THIS IS AN OPS POSITION.

The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children age 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. There is no charge or co-payment for services provided through the Agency.

Background Screening Requirement

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.

No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.

Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement, and Federal criminal records checks through the Federal Bureau of Investigation, and may include local criminal records checks through local law enforcement agencies.

DUTIES:
Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but is not limited to, always acting in a responsive, professional, courteous manner with the employees, customers and public we serve at all times.

This is a professional Other Personal Services (OPS) position with the responsibility of conducting evidence-based assessments for the Agency for Persons with Disabilities using the Questionnaire for Situational Information (QSI) for all individuals enrolled on the I-budget waiver, Consumer Directed Care Plus (CDC+) waivers as well as individuals on the APD waitlist.

SHSPA shall contact all parties, support coordinators, guardian, and consumers to set a time convenient to the consumer to conduct a QSI Questionnaires for Situational Interview out on the field in Dade and Monroe County.

SHSPA shall review supporting documents from Behavioral Analyst and Medical Case notes plans and provide case notes of their findings.

SHSPA shall enter all the information gathered from the QSI (Questionnaires for Situational Interview) and input it online to get a QSI score.

SHSPA shall review support plans and cost plans from support coordinators to conduct a QSI (Questionnaires for Situational Interview).

SHSPA attends and participates in Support Coordinator’s monthly meetings, workshops, staff meetings and other trainings as instructed.

SHSPSA will respond to all inquiries from consumers, guardians, support coordinators and provider agencies promptly.

SHSPA shall submit all the travel mileage by the 5th of the month from the previous month.

SHSPA shall comply with all DS/HCBS rules, FSL/HCBS rules, guidelines, Provider Rates and Region/Agency specific Policies & Procedures for support plans, cost plans and addendums.

SHSPA will assist with special assignments when needed.

Knowledge, skills, and abilities, including utilization of equipment, required for the position:

Must have ability to learn the QSI tool, QSI history and its use within APD along with scoring and interpretation of QSI items.
Ability to learn and apply the person-centered process used in conducting interviews for QSI.
Ability to input information on paper form as recommended during the QSI interview.
Must be available to work evenings and weekends.
Knowledge of the QSI tool: history and use within APD; scoring and interpretation of QSI items.
Knowledge of the person-centered process used in conducting interviews for QSI; includes the ability to demonstrate this process for new assessors and to evaluate for recertification purposes.
Ability to use computer-based systems, including but not limited to Microsoft (Word, Excel, Power Point, Share Point) and APD software such as ABC, iConnect and QSI Online Tool.
Ability to communicate effectively in face-to-face, electronic, and written formats.
Ability to establish and maintain effective working relationships with consumers, families, providers, and staff within the region and statewide.
Ability to prioritize and schedule work efficiently, determining work priorities and ensuring completion of work assignments
